DTES Women’s Intensive Case Management Team launches
On February 1, 2017, Vancouver Coastal Health launched a new Intensive Case Management Team (ICMT) for women in the Downtown Eastside.
The new ICMT is a multidisciplinary team of health care providers including social workers, nurses, nurse practitioners, a peer with lived experience, and health care workers, with access to other resources as needed.
ICMT clients are women who have complex care needs and face multiple barriers when accessing care. This includes, but is not limited to, concurrent disorders of problematic substance use or chronic dependence and mental illness, complex trauma, psychosis, and/or unmet chronic health needs, complicated by challenges related to social needs, safety and housing, among others. The service is delivered through a health outreach model to literally meet clients where they are – whether a shelter, residence or around the neighbourhood.
ICMTs are also currently in place to serve inner city youth, clients within the justice system, and both men and women within the DTES. The Women’s ICMT is made possible through a $3 million donation to the VGH and UBC Hospital Foundation.
